We Offer:

  • Free, year long health program for Native Hawaiians

  • Program eligibility requirements are Native Hawaiians in seek of support with diabetes, hypertension and/or obesity. Participants need to be a patient at KOKO

  • Quarterly health screenings, individualized health plans, nutrition & fitness support

  • Connection to community support and health coach 

  • This program was made possible thru funding provided by HMSA Foundation, OʻNeill Foundation, and County of Hawaiʻi  grants.

The Ulu Laukahi Project is designed to help our Native Hawaiian clients achieve a greater measure of health and well-being. We will inspire, inform, instruct, and help participants to successfully integrate lessons into their lifestyle. We aim to help clients reduce their weight to healthier Body Mass Index (BMI) levels, decrease and stabilize blood pressure to healthier levels, reduce and stabilize blood sugar levels to healthier levels; decrease psychological stress and dysfunction (depression, anxiety, substance abuse) to more functional levels, increase understanding and ability to prepare and eat healthier foods, and increase understanding and usage of health-promoting self-care activities in daily life. The project will increase engagement in family health and well-being and increase patient access and engagement with preventive/intervention services and care using culturally informed approaches at every step. Encouraging access to prevention and intervention services and promoting healthy, active, lifestyle changes will reduce the likelihood of chronic disease manifestations and progressions.
